Out-of-Network:
For all other insurance companies, I provide monthly superbills for clients who would like to seek out-of-network reimbursement. A superbill is an itemized document with required information to create a health insurance claim.
Understanding your Insurance Plan
It is important for you to understand your insurance plan’s mental health benefits before your first appointment.
Questions to ask your insurance company*:
Do I have benefits for outpatient mental health services?
If so, what is the coverage amount per session (CPT code 90791, 90837)?
Do I have a copay/coinsurance?
Do I have a deductible for mental health services, and has it been met?
Is there a limit to the number of therapy sessions per year that my plan covers?
Do I need a referral or preauthorization for psychotherapy?
*If you are using insurance other than First Choice Health, Premera Blue Cross, or LifeWise, please ask these questions in reference to out-of-network mental health benefits.
good faith estimate
Under the law, health care providers are required to give patients who don’t have insurance or who are not using insurance an estimate of their medical bills, including costs for items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare items or services, including psychotherapy, medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, or any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
Make sure you save a copy or picture of your Good Faith Estimate.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
For more information about your rights to a Good Faith Estimate or how to dispute a bill, visit www.cms.gov/nosurprises or call 1-800-985-3059.
